Cloud-Native Physical Therapy & Rehabilitation Platform on AWS

Architecting a Clinician-Developed, Globally Scalable Rehabilitation Platform for 10,000+ Patients Across 50+ Countries

Curovate is a clinician-developed, evidence-based physical therapy app that enables patients recovering from ACL surgery, knee replacement, hip replacement, and knee osteoarthritis to complete structured, stage-based rehabilitation programs at home — guided by video exercises, at-home range of motion measurements using just their phone, daily progress tracking, and direct access to licensed physical therapists via in-app chat and video sessions.

Since its launch in 2016, Curovate has been trusted by 10,000+ patients across 50+ countries, achieving a 68% exercise adherence rate vs. the 20% industry average for in-person physical therapy. Eternal Web Pvt. Ltd. architected and deployed the complete AWS cloud backend infrastructure powering the Curovate platform globally.

About Curovate: Curovate Inc. is based in Toronto, Ontario, Canada. The platform is led clinically by Nirtal Shah, PT, DPT, MScPT, MPH — with 25 years of clinical experience at University of Toronto's David L. MacIntosh Sport Medicine Clinic and 6 peer-reviewed publications in digital health. Curovate is a member of the NVIDIA Inception Program, AgeWell NCE, University of Toronto Health Innovation Hub (H2i), and CARP (Canadian Association of Retired Persons).

Business Challenge

Patient Adherence & Recovery Crisis: Only 20% of patients adhered to prescribed post-surgery rehabilitation due to lack of structured at-home guidance, no accountability, and no daily progress tracking.

Generic fitness apps provided no surgery-specific exercise protocols, no stage-based progression, no range of motion tracking, and no access to licensed physical therapists — leading to inconsistent recovery outcomes.

Recovery Timelines 40–50% Longer Without Structured Rehabilitation: Patients without a structured, daily guided rehabilitation program experienced significantly longer recovery times after ACL surgery, knee replacement, and hip replacement.

No At-Home Clinical Measurement Capability: Critical range of motion measurements could only be obtained during in-person clinic visits, leaving patients without objective progress data and unable to self-monitor their recovery between appointments.

Rehabilitation Access Limited to Clinic Visit Schedules: Patients in remote areas, those unable to travel post-surgery, or those with limited insurance coverage had no access to structured physical therapy guidance between appointments.

No Pre-Surgery Prehabilitation Programs Available: Patients preparing for ACL reconstruction, knee replacement, and hip replacement had no structured at-home strengthening programs to optimize surgical outcomes.

PT Communication Limited to Clinic Hours: Patients had no way to reach a licensed physical therapist between appointments for questions, concerns, or exercise clarification during recovery.

Platform & Infrastructure Challenges: The platform needed to reliably serve patients across 50+ countries with consistent video streaming performance and real-time app responsiveness, while securely ingesting external clinical data and maintaining enterprise-grade encryption and access controls for sensitive patient health records.

Solution Overview

Eternal Web Pvt. Ltd. architected and deployed the complete AWS cloud backend for Curovate — a secure, globally distributed, serverless-first infrastructure replacing the fragmented generic app experience with a clinically structured rehabilitation platform delivering surgery-specific protocols, at-home ROM measurement, stage-based progression, real-time PT access, and daily patient engagement to 10,000+ patients across 50+ countries.

  • Compute & API Layer — Amazon API Gateway & AWS Lambda: RESTful APIs manage all patient-app communications including exercise data sync, ROM measurements, progress tracking, PT chat, and authentication flows. Serverless Lambda functions handle exercise progression logic, stage advancement, ROM data processing, and PT session scheduling. Amazon EC2 with Auto Scaling and Elastic Load Balancing handles global patient load across time zones.
  • Data Storage & Management — Amazon RDS & S3: Amazon RDS serves as the primary relational database storing patient profiles, surgery types, rehabilitation protocols, exercise history, ROM measurements, progress milestones, and PT session records. Amazon S3 provides secure storage for physical therapy exercise videos, patient progress data, clinical resources, and rehabilitation documentation.
  • Global Content Delivery — CloudFront, Route 53 & ACM: Amazon CloudFront delivers physical therapy exercise videos with low latency to patients across 50+ countries, ensuring smooth, uninterrupted video-guided exercise sessions regardless of location. Route 53 provides high-availability DNS routing; AWS Certificate Manager enforces TLS encryption for all patient data in transit.
  • Patient Engagement & Notifications — Amazon SNS & SES: Multi-channel push notifications via SNS drive daily exercise reminders, progress milestones, PT chat alerts, and rehabilitation goal achievements. Amazon SES handles transactional emails for PT session confirmations, weekly progress reports, and subscription notifications.
  • Security & Access Controls — Cognito, KMS & Secrets Manager: Amazon Cognito manages patient and PT authentication with secure login, session management, and role-based access control. AWS KMS provides encryption keys protecting all patient health data at rest. AWS Secrets Manager handles secure storage and automatic rotation of database credentials and API keys.
  • Monitoring & Operations — Amazon CloudWatch: Real-time platform monitoring, API performance tracking, and automated alerting for service health across the entire Curovate infrastructure.

Measurable Results

240% Improvement in Exercise Adherence — Patient adherence improved from 20% (in-person PT industry average) to 68% on Curovate, directly addressing the core problem of patients failing to complete post-surgery rehabilitation.

Post-Surgery Recovery Time Reduced by 40–50% compared to patients using generic fitness apps without structured clinical protocols. Surgery-specific stage-based programs for ACL, knee replacement, and hip replacement deliver clinically validated progression.

92% Patient Satisfaction Rate achieved across ACL surgery, knee replacement, hip replacement, and knee osteoarthritis patients.

3,500+ At-Home Clinical ROM Measurements Monthly — ROM measurement accuracy validated at 0.998 correlation with professional physical therapist measurements (published in the International Journal of Sports Physical Therapy), confirming clinical-grade accuracy for patient self-monitoring.

Licensed PT Access Expanded from Clinic Hours Only to Daily via in-app chat and video sessions, reducing patient anxiety, preventing incorrect exercise execution, and improving recovery confidence.

10,000+ Patients Served Across 50+ Countries — Rehabilitation access expanded globally with AWS CloudFront CDN enabling low-latency exercise video delivery and real-time app performance worldwide.

Enterprise-Grade Platform at $1,000/Month AWS Infrastructure Cost — Serverless-first AWS architecture supports multiple clinical revenue streams including monthly subscriptions, annual plans, Video PT packages, and employee benefits programs — all without infrastructure redesign.
